Once you’ve received a completed Imaging Order Form from your doctor:
Call to schedule an appointment that is convenient.
We will ask you to carefully read the information on your Imaging Order Form to us over the phone.
We will then provide you with an estimate of fees and the approximate length of time for your appointment.
About Your Visit
BRING YOUR COMPLETED IMAGING ORDER FORM TO YOUR APPOINTMENT. Dentists refer patients to RMOX by completing one of our Imaging Order Forms, which indicates specific details and imaging requirements. You must bring the Imaging Order Form with you to your appointment.
PAYMENT IS DUE AT YOUR APPOINTMENT You will receive an estimate of fees when you schedule your appointment. We accept cash, check and credit cards. Although we are not dental providers for insurance plans, you will receive a receipt to submit to your insurance company for reimbursement per your individual policy. We are not Medicare or Medicaid providers.
YOUR IMAGES WILL BE SENT DIRECTLY TO YOUR DOCTOR. Your images will be sent directly to your prescribing doctor electronically or via USPS within a few days of your visit.
